“Allahumma musa-ghiral kabiir wa mukabbiras saghiir, sa-gghir maabi”

:::the Last Lecture:::

halloo..i'm still alive..
before lectures on Gastrointestinal finish, i would like to deliver a last lecture, CEH, no lahh..kidding meh..(cam bess sangat myself.ahaha!)
this is a sharing, non-tragic of a misery  story .actually, was recommended by my surgeon/lecturer on last week.

On September 18, 2007, computer science professor Randy Pausch stepped in front of an audience of 400 people at Carnegie Mellon University to deliver a last lecture called “Really Achieving Your Childhood Dreams.”
With slides of his CT scans beaming out to the audience, Randy told his audience about the cancer that is devouring his pancreas and that will claim his life in a matter of months.
On the stage that day, Randy was youthful, energetic, handsome, often cheerfully, darkly funny. He seemed invincible.
But this was a brief moment, as he himself acknowledged.
Randy’s lecture has become a phenomenon, as has the book he wrote based on the same principles, celebrating the dreams we all strive to make realities.

Sadly, Randy lost his battle to pancreatic cancer on July 25th, 2008, but his legacy will continue to inspire us all, for generations to come.

(he was soo energetic yet so funny.memang kelaka dowh lecture dia.haha!!)

:::What things that really triggered me,when he said this :

"And the other thing about football is we send our kids out to play football or soccer or swimming or whatever it is, and it’s the first example of what I’m going to call a head fake, or indirect learning.

We actually don’t want our kids to learn football. I mean, yeah, it’s really nice that I have a wonderful three-point stance and that I know how to do a chop block and all this kind of stuff.

But we send our kids out to learn much more important things. Teamwork, sportsmanship, perseverance, etcetera, etcetera.

And these kinds of head fake learning are absolutely important. And you should keep your eye out for them because they’re everywhere."

---------don't you think this about perfectly correct in our real life?? yeah, all parents absolutely wanted their children to be multi-learner like a multiplayer.haha.. children look very talented of course. but on the other hand, do these really raising them(the children) up?? (sorry, for my improper english.hehe)

"Almost all of us have childhood dreams; for example, being an astronaut, or making movies or video games for a living. Sadly, most people don’t achieve theirs, and I think that’s a shame. I had several specific childhood dreams, and I’ve actually achieved most of them.  More importantly, I have found ways, in particular the creation (with Don Marinelli), of CMU’s Entertainment Technology Center of helping many young people actually *achieve* their childhood dreams."

----there! he mentioned that it's ashame if we can't achieve our specific childhood dreams. Oh, alhamdulillah almost achieve all of my dreams, including my ambition. So, WAke up Aena!! fighting for the best for your future. Actually, sometimes, i don't realize that i'm on the path to be a doctor. kadang2 ketuk kepala tnya diri sendiri medical student ni nanti kerja ape yekk??-soalan stup*d i've questioned. but yea, kadang2 rasa seperti masih berada di takuk yang lama. Perasan secondary school lagi walaupun badan pam-pam mcm mak orang. Anyway, lots of complaints pun tarak guna,.With hikmah,we shall find a good solution..insyaALLAH..

ouhh! back to the topic,hehe..talking about his cancer, for our  information, pancreatic cancer is the 4th leading cause of cancer death in the United States. In 2008, an estimated 37,680 Americans will be diagnosed with pancreatic cancer, and 34,290 will die from the disease.
Early diagnosis for this particular type of cancer is extremely difficult, and there is no cure.

When you are touched by pancreatic cancer, your life changes in unimaginable ways.

Carnegie Mellon Professor Randy Pausch,
who was dying of pancreatic cancer, gave his last lecture at the university Sept. 18, 2007, before a packed McConomy Auditorium.
From 1988 to 1997, he taught at the University of Virginia.
He was an award-winning teacher and researcher, and worked with Adobe, Google, Electronic Arts (EA), and Walt Disney Imagineering, and pioneered the non-profit Alice project.

My Second Patient ::Clinical teaching 2

Assalamualaykum... hai semua..!!! haha mood sangat baik sekarang..alhamdulillah sudah selesai pun 1st exam paper-TITAS. Now boleh focus untuk exam medic pulak. not so sikit..aduii laaa.. OOh yea, sudah pun selesai clinical teaching yg kedua~ haha.. meaning, student medic belum lagi kecutian yea. Doakanlah kami banyak-banyak yea..ameen!

So, nak oyak lah sikit-mikit pasal my second patient neh.. is a sharing what...this time i did history taking, not physical examination ..(tetibe rase malas nk belatih PE GIT huhuu....)

A 80 years old Malay Female, was diagnosed gastritis about 30 years ago, and has been known to be hypertensive on past 4 years, came to HUSM ,presented with localised right iliac fossa pain,11 hours prior to admission..

if talking about 'right iliac fossa pain' probably, doc will think of something may related to appendix, ovary. more specifically acute appendicitis or ectopic pregnancy or any tumor at organ in that region. but the thing is, actually, provisional diagnosis is intestinal obstruction.must be pain at lumbar or umbilical region. so, i think the patient maybe wrongly pointed her finger when i asked her which part of her abdomen  did she really  felt the pain? (hahaha ..sebenanye agak challenging tau nak ambil history disease dri patient kelantan yang agak terherot telinga Aena cuba faham dialek perkataan diorang..hoho..banyak ngomel tul budak neh.)

ok.knowing the disease:
intestinal obstruction(halangan pada usus) is a blockage of your small intestine or colon that prevents food and fluid from passing through. Intestinal obstruction can be caused by many conditions, but it's most often the result of fibrous bands of tissue in the intestine (adhesions), hernias or tumors.

Intestinal obstruction can result in an array of uncomfortable signs and symptoms, including abdominal pain and swelling, nausea, and vomiting.

If left untreated, intestinal obstruction can cause the blocked parts of your intestine to die (become necrotic). This tissue death can lead to perforation of the intestine, severe infection and shock. However, with prompt medical care, intestinal obstruction can often be successfully treated.

Signs and symptoms of intestinal obstruction include:( Aena kasi sikit terjemahan lah sebab nak pembaca blog boleh ngerti n alert,kot-kot ada alami simptom/sign disease ni.learn the knowledge sama2 k!!!)
  • Crampy abdominal pain that comes and goes (intermittent)--(sakit kat abdomen kejap ada, kejap hilang, kejap rasa balik)
  • Nausea(pening2 rase macam nak muntah, tapi belum muntah..)
  • Vomiting or diarrhea(muntah2/cirit birit)
  • Inability to have a bowel movement or pass gas( tak boleh kentut!!)
  • Swelling of the abdomen (distention)-(ada kembung2/bengkak  kat kawasan perut/bawah perut)
  • Abdominal tenderness(rasa sakit kat kawasan abdomen)
  • Fever(demam laa..apa lagi maksudnye kan..)
ok , mesti ada yang keliru lagi kot, what actually abdomen really is???ok see this pic. bahagian tanda '+' tu lah abdomen yea..

so, When to see a doctor????
Because of the serious complications that can develop from intestinal obstruction, you should seek immediate medical care if you develop the signs or symptoms listed above...
note: dengan penuh ketar-ketar pukul 9.55 lari marathon ke HUSM(hosp USM), pergi naik lift untuk ke wad 7. bengang ramai gak. then, kat tingkat 3 ada orang masuk sembang2 menambahkan lagi ke'blurr'an daku. sampai2 dlm bilik kat wad 7, ohhh then doc came in. Oh my GOD!! he was in the lift just nw! dia sengih2 aja,n me lagi la sengih2..present macam ayam berak kapur je..ahahaaaa..

tata lah k~

!!!...pooping out..!!!

come again....talking about those craziness that i've done.
ok not this time. i always like this! huh.a bidan terjun saat akhir. mksudnya dengan otak+minda yg blurr terus terima saja apa yang orang suruh. ok. itu masalah diri sendiri.,tapi kenapa nak bising2 kat blog??? jwapannya : i'm expressing myself. yeah!! that's great ok!!

ok, 1st i dun knw how come me and my partner for clinical teachng are going to present this week. err, bukan ada turn orang lain dulu meh...???
--- not funny, dah ambil medical, proceed ajalah minah oii.nanti oncall banyak-banyak lagi teruk kau kena. ok dah tak nak banyak complain. me also not perfect.

then,hu why pulak lah got wrong no for doctor yang akan in charge on my clinical teaching this week..?? ini bikin saya stress bila orang tu cakap: i think u got wrong no!
--must follow up to pejabat akademik ok!

tiada hati mau ketawa walau sedikit.
Hai mak, call adik, i miss u!!

note: Red words are my complaints, below are my answers.hhaaahahaa
